- The name you got banned under: (In-game character name, forum account name, or Discord username)
- Your SteamID or a link to your Steam profile: (If applicable)
- Information about the ban:
- When were you banned? (Please include date and approximate time)
- How long is the ban? (Temporary / Permanent / Unknown)
- Who applied the ban? (If known)
- What is the ban reason? (Copy the reason shown on the ban message or from the MelonRP Ban Panel)
- Do you admit to what you were banned for? Please give your honest perspective.
- Why should your ban be lifted or reduced? (Explain what you’ve learned and how your behavior will improve)
- Have you been banned or warned before? (Check your record on the MelonRP Panel)
- Can you provide any screenshots, logs, or evidence that supports your case?
- List of people involved who can provide input: (Optional, but useful for verification)
